raayjournal.ir با تصویب قانون بودجه سال 1398، مفاد بند‌ «ی» تبصره 6 که صاحبان حرف و مشاغل پزشکی، پیراپزشکی، داروسازی و دامپزشکی را مکلف به استفاده از پایانه‌های فروشگاهی در چهارچوب آیین‌نامه تبصره ۲ ماده ۱۶۹ قانون مالیات‌های مستقیم نموده، مورد توجه بسیاری از دادستانی‌های سراسر کشور قرار گرفته و آنان در راستای اجبار مکلفان به اجرای قانون، اقدام به اصدار دستوراتی نموده‌اند. ورود دادستانی‌ها به این مسئله، از حیث اهمیت زاید‌الوصف مالیات برای مصلحت عموم جامعه و ایجاد مشکلات احتمالی ناشی از حمل وجه نقد جهت پرداخت به مراکز خدمات درمانی، با موضوع احیای حقوق عامه مرتبط است. این نوشتار در صدد است با رویکردی تحلیلی، ابعاد حقوقی ورود دادستانی‌ها به مسئله‌ی الزام مراکز خدمات درمانی به استفاده از پایانه‌های فروشگاهی را از منظر حقوق عامه نقد و ارزیابی کند. عدم وجود شبکه ارتباطی میان دادستانی‌های سطح کشور و لزوم اقدام مشابه آنان در پرونده‌های مشترک ملی و منطقه‌ای، خلأ نظام حقوقی موجود در حیطه‌ی ضمانت‌اجرای اقدامات و دستورات ماهیتاً اداری دادستانی و لزوم تقویت آن، عدم جامعیت مخاطبان دستورات، الزام به استفاده از پایانه فروشگاهی نه صرفاً الزام به نصب آن و لزوم مستند، مستدل و شفاف بودن این نوع دستورات در جهت تسری بعضی آثار و نتایج آراء و احکام دادگستری بر آن‌ها، از یافته‌های تحلیل حاضر است.: 220 days ago

Rising incidence of aggressive Cesarean Scar Pregnancy (CSP) lacks consensus treatment. This paper aims to evaluate the use of methotrexate in reducing bleeding in the management of CSP. Three cases of CSP, each managed with a distinct approach, are presented in this study. The first patient was diagnosed at 13 weeks of gestation. A hyst***omy was performed because of massive bleeding. The second patient had repeated CSP. The first CSP was diagnosed in six weeks, and curettage was performed. The patient had been bleeding for three months. CSP in the subsequent pregnancy was diagnosed at five weeks of gestational age, and the patient decided to terminate by systemic administration of methotrexate twice, followed by resection. Bleeding was minimal. Pregnant women must be screened as early as possible, especially in patients with a history of uterine wall injury, to rule out CSP. Methotrexate gives a better result for CSP management.: 220 days ago
